Step#04: Candidate can pay the prescribed fee through Bank Challan available at “fms.uhs edu.pk” at any Bark of Purnjab (BOP) branch.
Step#05: Applicants have to attach the bank challan with the application form. Candidates will be required to submit a fee of Rs.2000/- for BPS 18 and above posts.
Step#06: The original paid challan along with the printout of the admitted form, attested copies of educational certificates, testimonials, CNIC, domicile and passport size color photograph must reach the required address by post or hand.
